Having tools to assess the prognosis in patients with traumatic brain injury (TBI) is essential for an individualized therapeutic management. The neutrophil to lymphocyte ratio (NLR) is a widely used inflammatory biomarker that has proven to have a prognostic ability in various neurological diseases including TBI. However, there is no systematic review that summarizes the available evidence. We conducted a literature search in the 6 databases. The quality of the studies was assessed using the Newcastle-Ottawa Scale (NOS). The effects were expressed as odds ratios (ORs) and their 95% confidence intervals (CIs). Standard mean differences were converted to Log[OR] using Chinn’s method. The quantitative analysis was performed using a random effects model. Small study effects were assessed with the Egger's test. We included 8 cohort studies. The results showed no association between NLR values and mortality in patients diagnosed with TBI (OR 2.42; 95% CI 0.63 to 9.28; p=0.20; I2=92%) or with favorable/unfavorable outcomes in patients with TBI (OR 3.00, 95% CI 0.76 to 11.85; p=0.12; I2=99%). The subgroup analyzes showed no association (Early Outcome: OR 2.63, 95% CI 0.99 to 6.96; p=0.05; I2=95%) (Late Outcome: OR 3.36, 95% CI 0.22 to 52.17; p=0.39; I2=100%). There was no indication of small study effects for favorable/unfavorable outcome (Egger’s test=0.149). We found no association between NLR values and favorable/unfavorable outcome or mortality. More high-quality studies are required to have a better perspective of the prognostic capabilities of NLR in TBI.
