INTRODUCTION: Depression affects physical health, productivity and life’s quality. Probiotic consumption could prevent or relieve depressive symptoms by modulating the gut-brain axis. However, today’s evidence is conflicting. OBJECTIVE: To summarize the evidence about the efficacy and safety of probiotic consumption in reducing depressive symptoms in adults. MATERIALS AND METHODS: Systematic review of randomized clinical trials identified in Medline, Web of Science, CENTRAL, LILACS, and gray literature sources through November 12, 2021. Results were combined by meta-analysis in Stata V.16. Risk of bias was assessed using the Cochrane Collaboration tool. Publication bias was analyzed using funnel plot diagrams and Egger's test. RESULTS: Sixteen studies with 1292 participants were included. Probiotic supplementation reduced depressive symptoms (mean difference [MD]: -0.19; 95% CI: -0.30 to -0.08; I2=0%; p<0.01). This reduction occurred mainly in Asian studies, when the BDI and HDRS scales were used, when multiple kinds of probiotics were administered, for more than eight weeks, in doses greater than 10x109 CFU and before food. All studies had low risk of performance bias and random sequence generation, and most had low risk of allocation concealment. No publication bias was evident. CONCLUSIONS: Probiotic consumption reduced depressive symptoms in adults. Further evidence is needed to identify the optimal composition and mode of administration of probiotics, and to assess whether microbiota status may influence their efficacy.
