Introduction The expression of sexual orientation and gender identity can affect mental health in the lesbian, gay, bisexual, transgender, and intersex population. Method Observational, analytical, cross-sectional, and retrospective study. We used as a secondary source the First Virtual Survey on the LGBTI population, made by the National Institute of Statistics and Informatics. Prevalence ratios (PR) and 95% confidence intervals (95% CI) were obtained by Poisson regression models. Result 11 345 people were eligible. The median age of the participants was 25 years (IQR: 21-30). 43.20% of people indicated that they were gay, 22.45% lesbian and 26.03% bisexual. Regression models showed a statistically significant association between the expression of sexual orientation and/or gender identity and mental health (PR: 0.83, CI: 0.76–0.90, p < 0.001). Conclusion Individuals who expressed their sexual orientation and/or gender identity were 17% less likely to have mental health problems.
