Introduction: According to the World Health Organization (WHO), iron deficiency anaemia affects 800 million children under the age of five worldwide. In Latin American, several strategies have been developed to eradicate childhood anaemia, such as iron supplementation and promotion of fortified foods. Despite this, some qualitative studies continue to report the presence of barriers and facilitators that could influence adherence to treatment with iron supplements. Objective: To synthesize the available evidence from qualitative studies that investigated the barriers and facilitators of iron supplementation in children under 3 years of age in Latin America from the perspective and experience of caregivers and health personnel. Methods: Qualitative systematic review of literature published during the period 2010-2020. A literature search was conducted in PubMed, SciELO, Scopus, MEDLINE, LILACS, Wiley Online Library and Google Scholar. Methodological quality was assessed using the CASPe Program scientific assessment criteria and standards for reporting qualitative research (SRQR) and thematic synthesis was applied for data analysis. Results: Thirteen of 1538 results were selected from the review of scientific articles. Twenty-eight barriers and 24 facilitators were identified from the perspective of caregivers and health personnel or professional. The main barriers corresponded to the dimension of characteristics and actions of the caregiver and health personnel. Most of the facilitators identified were related to the caregivers' point of view, the influence of the social environment and government support. Conclusion: The synthesis indicates that the strategies to improve the acceptability and perception of health improvement in the children by caregivers facilitate the continuity of supplementation, while the adverse effects and negative comments of the supplement received from the family or social environment contribute to limit adherence to iron treatment.
