The first stage of this research uses a Stochastic Frontier Analysis (SFA) to determine the efficiency of public health expenditure at regional level for the period 2009-2018 based on the maximization of a single output subject to a single input. Five appropriate outputs representing the Peruvian health system were identified on the basis of the literature consulted: life expectancy at birth, under-five mortality rate, infant anemia rate, access to prenatal care and skilled care during delivery. Through factor analysis, an indicator that synthesized these products into a single health outcome indicator was developed. Likewise, health expenditure at regional level for the period 2009-2018 is considered as the main input. The results revealed that a major expenditure on health at regional level, does not necessarily reflect in an improvement in health outcomes. There are regions that with little investment have positioned themselves among the best performers, while the opposite is also true. In the same way, throughout the analysis period, there are changes in the efficiency level of the regions, as well as in health outcomes. In a second stage, some determinants of the level of efficiency found for the regions were identified through a Tobit model for panel data. Regional GDP and the level of urbanization were considered as exogenous variables that could affect the item. The results show that the level of urbanization is a relevant variable and that it positively affects the efficiency indicators in the 2009-2018 period.
