Background: Obesity is a public health problem worldwide, the figures are also increasing at the level of Latin America and developing countries such as Peru with very striking figures. Within the causality that explains this problem there are different associated factors, according to different bibliography, sex works as an effect modifier for the association between sleep duration and obesity. Therefore, the objective of this work was to evaluate if there is a relationship between these variables, and if this association differs according to sex. Methods: A cohort study was conducted, using secondary information from the database of the Millennium Children study. The outcome variable was obesity, based on BMI for age according to the z-score; the exposure was the child's sleep duration (reported by parents) categorized using the National Sleep Foundation classification, as a numerical variable. To calculate the relative risk, the Poisson regression with robust variance was used, taking into account the changes of the result variable over time. The mixed linear model was used when the outcome and the exposure variable were categorized as numerical variables, taking into account the variation in time for both. Results: Data from 1949 children were analyzed, with an initial mean age of 4.3 years (standard deviation: 0.3) and 962 (49.5%) women. The short duration of sleep was present in 26.0% (95% CI: 24.0-28.0) at the beginning of the study. After 9.6 years of follow-up, the incidence of obesity was 0.83 (95% CI 0.70 - 0.98) per 100 people-years at risk. In the multivariate model (n = 1579), there was no association between short duration of sleep and obesity in the entire sample (n = 1579, RR = 1.00; 95% CI 0.71-1.41; p = 0.13) but the risk of obesity was lower among girls, (n = 816; RR = 0.45; 95% CI: 0.21-0.96; p = 0.03) compared to boys (n = 763; RR = 1.43; 95% CI: 0.95-2.14; p = 0.09). On the contrary, each additional hour of sleep was associated with an increase in the average BMI of boys (0.05; 95% CI: 0.02-0.08; p <0.001), but not among girls (- 0.02; 95% CI: -0.05 to 0.01; p = 0.11) Conclusions: Our results showed a lower risk of obesity due to the short duration of sleep in girls compared to boys. Each additional hour of sleep was associated with an increase in BMI in boys by 0.05 but not among girls. It is evident that sex was an effect modifying variable.
