Objectives: To identify the factors associated with survival, immunological and virological failure in patients with HIV in HAART (Highly Active Antiretroviral Treatment) in a National Reference Hospital in Lima, Peru. Materials and methods: Retrospective cohort observational study with data from patients over 18 years of age who started highly active antiretroviral therapy (HAART) during the years 2010 - 2015 and who had at least 2 years of follow-up. Virologic failure was defined as a viral load value above 400 copies/ml after six months on HAART. Immune failure was defined by the absence of elevated CD4 count in at least 50 cells per ml in a year of treatment. Results: 893 records were analyzed, the median age was 40 years (RIC 33-48), the male sex was the most frequent (75%), 69% had Efavirenz-based HAART and the sexual practice that was more frequent was heterosexual (66%). The factors associated with lower mortality were Efavirenz-based HAART (HR = 0.46; 95% CI 0.31-0.67; p <0.01), high baseline CD4 (HR = 0.60; 95% CI 0.44-0.83; p <0.01) and homosexual / bisexual sexual practice (HR = 0.63; 95% CI 0.45-0.87; p <0.01). The factors associated with the lowest probability of virological failure was Efavirenz-based HAART (RR = 0.54; 95% CI 0.35-0.83; p <0.01). The factors associated with the highest probability of immune failure were female sex (RR = 1.14; 95% CI 1.01-1.30; p = 0.04) and homosexual / bisexual sexual practice (RR = 1.20; 95% CI 1.06-1.36; p <0.01), while a higher baseline CD4 count was associated with a lower probability (RR = 0.99; 95% CI 0.86-0.94; p <0, 01). Conclusions: The factors associated with greater survival were Efavirenz-based HAART, a higher baseline CD4 count and homosexual / bisexual sexual practice. The factor associated with a minor virological failure was Efavirenz-based HAART. The factor associated with a lower immune failure was a higher baseline CD4. The design of estimated considerations to reduce treatment failure and mortality in these patients should be considered.
