Introduction: Colorectal cancer (CRC) is the fourth most common cancer worldwide, and in Peru, it is the second most frequent digestive cancer. Colonoscopy screening after 50 years of age has shown to reduce CRC mortality. However, access to screening is not homogeneous in the population, and health insurance could be an important determinant. Objectives: To assess whether there is an association between the type of health insurance and the performance of preventive colonoscopy in Peruvian adults aged 50 to 75 years, users of external consultation, in Peru. Methods: An analytical cross-sectional study was carried out using data from the National Survey of Satisfaction of Users in Health (ENSUSALUD) 2016. It was asked about performing preventive colonoscopy in the last ten years and the type. Prevalence ratios calculated and adjusted for potential confounders were calculated. Results: The average age was 60.5 years (s.d.: 6.8) and 54.8% were women. The prevalence of having performed a preventive colonoscopy was 11.8% [95% CI: 8.1% to 15.5%] In the adjusted analysis, patients with private insurance [RPa: 4.48; 95% CI: 1.18 to 18.63] and SSFFAAPP [RPa: 6.38; 95% CI: 1.66-24.63] were more likely to have had a colonoscopy compared to uninsured patients. No significant difference was found between SIS and EsSalud insurance. Conclusions: Patients with private insurance, insurance of the FF.AA. were more likely to have undergone a preventive colonoscopy compared to the uninsured. Meanwhile, the SIS insured, EsSalud and the uninsured, no statistically significant differences were found.
