Introduction: According to ENDES 2018, nationwide, 66.4% of children under six months have received exclusive breastfeeding (EBF). Several studies have shown the benefits of breastfeeding for the mother and the baby and qualitative studies find that the influence of family members, health personnel, friendships and perceptions on breastfeeding can be factors that affect the duration of the EBF. Objective: To explore the perceptions of exclusive breastfeeding and reasons for abandonment in first-time mothers over 18 years of age at the Maria Auxiliadora Hospital in the San Juan de Miraflores district. Methods: The study is qualitative and phenomenological design. Two in-depth interviews were conducted with key informants, 19 in-depth interviews and 2 focus groups of mothers, making a total of 31 first-time mothers with children between 1 and 7 months old. Results: The type of cesarean delivery causes a delay in the onset of EBF, the introduction of water and / or infusions begin from the third month on the perception of thirst, gas and constipation of the baby. The introduction of milk formulas is given from the week of the baby's birth due to the type of birth, low milk production, the perception of hunger of the baby, work or study of the mother and the contradictory messages they receive from of family members and health personnel. Conclusion: Caesarean delivery, late attachment, low breast milk production, breast problems, hunger, thirst, baby craving, gas and baby constipation, the reasons that the vast majority of mothers They mention to justify the introduction of water, infusions, milk formulas or meals before 6 months of age of their babies. Likewise, returning to work or studies, contradictory counseling and unimpressive advertising are also factors that generate the interruption or abandonment of the EBF.
