Introduction: In newborn infants with meconial aspiration syndrome (MAS) CPAP may be more beneficial compared to endotracheal intubation (ET). Aim: To compare the efficacy of CPAP versus TE in newborn infants with SAMe. Methods: Systematic review of randomised controlled trials (RCTs) in five databases. Two authors independently selected studies, extracted information and assessed risk of bias. Risk of bias was performed using the Cochrane collaboration tool. Primary outcomes were need for mechanical ventilation and pneumothorax; secondary outcomes were gestational age, birth weight, APGAR, cord pH and days of hospitalization. We used relative risk (RR) and mean difference (MD) to assess the effect on dichotomous and continuous outcomes, respectively. We used the Mantel-Haenzel (MH) fixed-effect model due to anticipated shortages of events. Results: We included three RCTs (n=432). CPAP significantly reduced the need for ventilation by 64% compared to TE (RR 0.36; 95% CI 0.18 to 0.71; p<0.004). CPAP had no effect on the incidence of pneumothorax compared to TE (RR 1.40; 95% CI 0.28 to 6.95; p<0.68). There were 0.53 fewer days of hospitalization in CPAP compared to TE (MD -0.53; CI95% -0.75 to -0.30; p<0.0001). Other secondary outcomes did not improve with CPAP compared to TE. Conclusions: The use of CPAP reduces the need for ventilation and days of hospitalization compared to TE with neonates with SAMe. Other outcomes were not improved by CPAP.
