The COVID-19 disease has produced a great change in health systems and societies worldwide. Research has evolved to a great extent. Despite the fact that so far a great susceptibility to the virus has not been demonstrated in neonates and infants, all health personnel have had to adapt to changes in resuscitation procedures and routine care in this particular population, to provide quality care during the prenatal and postnatal stages. In this review, we present the prevalence, characteristics, and transmission of COVID-19 in newborns and infants. Likewise, we discuss various care practices that have changed in the context of the current pandemic, addressing prenatal corticosteroid management, delivery room management, late cord clamping, skin-to-skin contact, and breastfeeding. In addition, the most recent changes regarding resuscitation in term and premature infants and newborns are described, with adequate airway management and ventilatory support; highlighting the most appropriate protection measures for health personnel, mothers, newborns and infants. The methodology is analytical, with a qualitative approach, taking as a sample various cases carried out at the national and international level through searches in the main databases such as PubMED, MEDLINE, Nature Communications, SpringerOpen, Scielo, Redalyc, Scopus and EMBASE. The free international data shared with researchers around the world such as: Centers for Disease Control and Prevention (CDC), the World Health Organization (WHO) and the Johns Hopkins Bloomberg School of Public Health related to COVID-19 is also reviewed. In the analysis, it is appreciated that the favorable knowledge regarding the impact of COVID-19 in newborns and infants is still restricted; however, it allows us to outline different scenarios that take into account the known risks and benefits in the neonate; and, the correct decision making after being analyzed in a reliable and perceptible way. The main conclusions emphasize a protocolized multidisciplinary care, preserving both exclusive breastfeeding and joint accommodation, following strict hygiene measures. The literature available to date shows a low rate of disease in newborns and infants, therefore, the different scenarios regarding primary care may change in the near future.
