Objective: To determine the association between testosterone levels and the risk of Type 2 diabetes mellitus in men. Material and methods: Using the PRISMA method, a search and collection of scientific articles from data sources such as Pubmed, Scopus, Web of Science, Embase and gray literature search engines, published from January 1980 to July 2022, meeting the inclusion and exclusion criteria, was performed. For their analytical analysis, meta-analysis was used to calculate ORs; heterogeneity was evaluated with Cochran's Q test and inconsistency index, with a significance level of p<0.10. Results: In this analysis of the association between hormone levels and Type 2 Diabetes Mellitus in men, it was evidenced that low levels of total testosterone correlate significantly with an increased risk of the disease (OR: 1.85; 95% CI: 1.31 - 2.61). In contrast, high levels of total testosterone proved to be a significant protective factor (OR = 0.67; 95% CI: 0.54 - 0.85). The presence of low values of sex hormone-binding globulin was also significantly associated with an increased risk of developing Type 2 Diabetes Mellitus (OR = 2.39; 95% CI: 1.04 - 5.05). No significant associations were observed with estradiol, dihydrotestosterone (DHT) or bioavailable testosterone levels. Conclusions: There is an association between testosterone levels and the risk of occurrence of Type 2 Diabetes Mellitus in males
