Objective: To determine the association between cofactors and high-grade squamous intraepithelial lesion according to the Pap smear (PAP) of the cervix in northern Peru. Material and methods: Case-control design, retrospective, analytical, with data collection of medical records of patients treated in outpatient clinic and hospitalization of the Gynecology service of the Cayetano Heredia Hospital in Piura whose PAP result was reported as high intraepithelial lesion (H-SIL) and low grade (L-SIL). The sampling applied was non-probabilistic. Results: 243 medical records were analyzed: 51 cases and 192 controls. GA-ILIs were more frequent in single women (25.5%), with low educational level (9.3%), whose personal oncological history other than cervical cancer (10.2%) and relatives compatible with cervical cancer (9.8%) were positive The clinical cofactors associated with the appearance of H-SIL were the early age of initiation of sexual intercourse (aOR: 6.80, 95% CI: 2.45 to 18.83), initiation of sexual intercourse in the first two years of menarche (aOR: 16.06, 95% CI: 3.87 to 66.57), menarche before 12 years (aOR 6.53, 95% CI 1.88 to 22.69),and previous history of STIs (aOR: 3.22, 95% CI: 1.15 to 9.02). . Conclusions: In the present research it was found that personal and family oncological history, as well as clinical cofactors are variables associated with the appearance of high-grade intraepithelial lesions in the outcome of PAP.
