To determine the association between knowledge and attitudes about COVID-19 vaccination in the population of Ayabaca, 2021. Methods: An analytical, prospective, cross-sectional, and observational research design was implemented. The study was conducted in the general adult population of the city of Ayabaca. General knowledge related to vaccines as a measure to face COVID-19 and its importance in prevention was evaluated, as well as attitudes about vaccination. STATA v.15 was used for statistical analysis. Results: A total of 345 participants over 18 years of age were recruited. Of the participants, 50.4% were female and 44.9% were aged between 18 and 30 years. Twenty percent of the participants had an adequate attitude towards vaccination against COVID-19. Among those with adequate attitude, a high level of knowledge about COVID-19 and vaccination predominated, while among those with inadequate attitude, a low level of knowledge about COVID-19 and vaccination predominated. An association was identified between the level of knowledge about COVID-19 and vaccination against COVID-19 and the attitude towards vaccination (p<0.001). Conclusions: Factors associated with attitude toward COVID-19 vaccination were knowledge about COVID-19 and knowledge about COVID-19 vaccination
