To compare the effectiveness and safety of phacotrabeculectomy and gonioscopy-assisted transluminal phacotrabeculotomy (FACOGATT) in patients with primary open-angle glaucoma. Methodology: A total of 212 medical records from June 2021 to June 2023 were analyzed using the nonparametric Mann-Whitney U test for comparison of medians for independent samples and the Wilcoxon test for dependent samples. Continuous data were expressed as standard deviation, median and median; categorical data were expressed as percentages and frequencies. For safety, a percentage of complications was established for each technique and analyzed by chi-square. To be considered statistically significant, a P value <0.05 was considered. Results: A total of 212 clinical histories were analyzed, obtaining that both techniques decrease intraocular pressures, Facotrabeculectomy preoperative mean of 21.11 mmHg and at the sixth postoperative month obtained 16.70 mmHg with a reduction of 4.42 mmHg (20%) and FacoGATT had a preoperative mean of 20.10 mmHg and at the sixth postoperative month obtained 13.74 mmHg with a reduction of 6.36 mmHg, with a significant statistical difference. Regarding safety, phacotrabulectomy obtained 21.70% and phacoGATT 9.43% of complication rate, being in both the most frequent complication, hypotony and hypotony. The number of medications was reduced in both groups with a significant difference. Conclusion: Both techniques are effective in reducing the mean intraocular pressure at the sixth postoperative month to less than 20% of the mean baseline pressure; both techniques are safe, the most frequent complications being hypotony and hypotension; the use of medications to reduce intraocular pressure decreases in both groups after surgery.
