Objective: To demonstrate that the prognostic nutritional index <41 is a predictor for complications in moderately severe acute pancreatitis at the Trujillo Regional Teaching Hospital during the 2014-2018 period. Material and methods: A retrospective, cross-sectional study was conducted in which 165 patients with moderately severe acute pancreatitis were included, which were divided into 2 groups: patients with complications with Nutritional Prognosis Index <41 or ≥41 which were compared with patients without complications where the same index was found; applying the odds ratio, and the chi-square statistical test. Results: Significant differences were found between the age and average scores of the APACHE II scale and regarding the frequencies of etiology, diabetes mellitus, arterial hypertension, obesity, hypercholesterolemia and BISAP score among patients with moderately severe acute pancreatitis with or without local complications (necrosis, infection, pancreatic abscess, pancreatic pseudocyst and peripancreatic hemorrhage). Prognostic nutritional index <41 is a predictor of complications in patients with moderately severe acute pancreatitis with an odds ratio of 40.9 which was significant (p <0.05). The main local complications in patients with moderately severe acute pancreatitis were infection, pancreatic abscess and pancreatic pseudocyst. In the multivariate analysis, the predictive factors for complications in patients with moderately severe acute pancreatitis were the prognostic nutritional index <41, having an average age of 67.6 ± 16.5 and the BISAP score greater than or equal to 3 points. Conclusion: The prognostic nutritional index <41 is a predictor for complications in moderately severe acute pancreatitis at the Trujillo Regional Teaching Hospital during the 2014-2018 period.
