Objective: To determine if the albumin-lactate ratio is a predictor of mortality in children with septic shock at the Hospital Belén de Trujillo. Material and methods: Observational analytical study of retrospective cohorts. In a population of 238 patients, the study sample consisted of 116 pediatric patients with septic shock treated at the Hospital Belén de Trujillo from 2015 to 2022, divided into two groups, an exposed cohort, 58 patients with a high lactate-albumin ratio and an unexposed cohort, 58 patients with a lactate-albumin ratio. albumin not elevated. The statistical analysis was carried out applying the Chi Square test, calculating the Relative Risk (RR), considering significance of 5% and its confidence intervals at 95%. Results: The frequency of children who died with septic shock who had an elevated albumin lactate ratio was 29.3% and those who had a non-elevated albumin lactate ratio was 12.1%. The elevated albumin lactate ratio was a predictor of mortality in pediatric patients with septic shock (p=0.002; RR=2.429 95%CI 1.09-5.413). Female sex (p=0.015), leukocytosis (p=0.004), plateletopenia (p=0.043), hyperglycemia (p=0.027), hyponatremia (0.004) and mechanical ventilation (p=0.000) were the characteristics general conditions that were associated with the mortality of pediatric patients. Conclusion: The albumin-lactate ratio was a predictor of mortality in children with septic shock at the Hospital Belén de Trujillo
