Objective: To demonstrate the efficacy of high doses of spironolactone in the clinical improvement of patients with decompensated heart failure. Methods: A pragmatic clinical trial was carried out in which 284 patients with a diagnosis of decompensated heart failure were included, according to selection criteria and divided into 2 groups: 142 patients treated with high doses of spironolactone and 142 treated with non-high doses of spironolactone, and the chi square and relative risk statistics were calculated. Results: Of the total number of patients with high doses of spironolactone, 79.6% (113 patients) showed clinical improvement; on the other hand, of the total number of patients without high doses of spironolactone, 69% (98 patients) showed clinical improvement; therefore, we say that high doses of spironolactone are effective in clinical improvement in patients with decompensated heart failure (RR = 1.153 CI 95% 1.004 - 1.324, p=0.042). In the multivariate analysis it was found that variables associated with clinical improvement were high-dose spironolactone (RR:1.832 p=0.045 , 95% CI 1.014 - 3.311), high-dose furosemide (RR:2.988 p=0.000 , 95% CI 1.674 - 5.336) ; and female gender (RR : 2.946 p=0.002 , 95% CI 1.505 - 5.767).Conclusion: High doses of spironolactone are effective in the clinical improvement of patients with decompensated heart failure
