Objective: To demonstrate whether the elevated platelet/lymphocyte ratio is a prognostic factor for postoperative morbidity in advanced gastric cancer after D2 lymphadenectomy. Materials and Methods:The study design was case (56 patients with postoperative complications) and controls (56 patients without postoperative complications) nested in a cohort, including a total of 112 patients who underwent gastrectomy with extended D2 lymphadenectomy for advanced gastric cancer. For the categorical variables, the Chi-square test, and logistic regression test was applied for the multivariate analysis. Results: In the bivariate analysis, there was a significant association between the elevated lymphocyte platelet ratio, clinical stage, and infectious postoperative complication (p=0.02; p=0-0271 respectively), however, no significant association was found between perioperative transfusion, comorbidity, procedure surgery, age and sex. In the multivariate analysis, a statistically significant association was observed between the elevated lymphocyte platelet ratio, the clinical stage and infectious postoperative complications (p=0.01, OR=3.14, IC=1.31 - 7.55 ; p=0.005, OR=0.18, IC=0.05 - 0.60 respectively). However, perioperative transfusion, comorbidity, surgical procedure, age, and sex did not have a statistically significant association with postoperative morbidity. Conclusion: Elevated platelet/lymphocyte ratio was associated with postoperative morbidity in patients undergoing gastrectomy with D2 lymphadenectomy for advanced gastric cancer
