To determine if the greater daily protein deficit in adult patients with critical COVID-19 is a risk factor for prolonged hospital stay in the intensive care unit compared to a lower daily protein deficit. Material and Methods: Longitudinal observational analytical study, retrospective cohort. Carried out at the HOSPITAL DE ALTA COMPLEJIDAD DE LA LIBERTAD ““VIRGEN DE LA PUERTA““ on critically ill adult patients with COVID-19 hospitalized in the ICU (intensive care unit) during the period from January to December 2021. The sample attended in 164 medical records of patients who met the selection criteria, including those with a diagnosis of critical COVID-19 at the HOSPITAL DE ALTA COMPLEJIDAD DE LA LIBERTAD ““VIRGEN DE LA PUERTA““ during the period from January to December 2021 and excluding adult patients with such a diagnosis who died within 48 hours of admission to the ICU and who presented incomplete data of interest. Patients admitted to the ICU with a positive COVID-19 test, whose reason for admission was an extrapulmonary pathology, were also excluded, as were those transferred from another hospital before 28 days or those who were admitted to the ICU from another hospital or clinic setting. receiving MV (mechanical ventilation) and nutrition. To establish the relationship between the study variables, the chi-square test was obtained, considering p < 0.05 as a significant difference; to estimate the strength of association, the relative risk (RR) was calculated, with its respective 95% confidence interval. Results: Since all the patients in the study sample presented daily protein deficit, they were separated into 2 groups: those with a higher daily protein deficit (<0.8g/kg/day) and those with a lower daily protein deficit (>= 0.8g/kg/day - <1.3 7 g/kg/day) and thus see its relationship with prolonged hospital stay. Of the total number of patients with a higher daily protein deficit, 72.2% had a prolonged hospital stay, while of the total number of patients with a lower daily protein deficit 75% had prolonged hospital stay, this difference was not significant (p=0.703). Regarding the intervening variables in the multivariate analysis, arterial hypertension had a statistically significant association with prolonged hospital stay, with a value of p= 0.001 and a RR of 11.68 (95% CI: 2.91-46.52) Conclusions: Higher daily protein deficit is not a risk factor for prolonged hospital stay in adult critically ill COVID-19 patients in the intensive care unit compared with lower daily protein deficit.
