Abstract: Determinar los factores asociados a mortalidad en los pacientes con
reintervenciones quirúrgicas abdominales de emergencia.
Materiales y método: Se realizó un estudio analítico observacional,
retrospectivo, de corte transversal. Se revisó 111 historias clínicas de pacientes
con reintervenciones quirúrgicas abdominales durante el 2016 a 2019, que
cumplieron con los criterios de inclusión y exclusión.
Resultados: La prevalencia de mortalidad en reintervenidos es de 11.71%. Los
factores asociados a mortalidad son edad (58,84 Vs 37,56; p<0,001); estado civil
soltero (61,5% Vs 24,5%; p=0,006); IMC normal (53,8% Vs 52%; p<0,001);
hemoglobina preoperatoria (11,30 Vs 13,55; p<0,001); valor de leucocitos al
ingreso (15594,07±5053,37 Vs 12869,32±2801,29; p=0,002), uso de
inotrópicos/UCI (53,8% Vs 0%; p<0,001), diagnóstico de intervención quirúrgica
de causa hemorrágicas (53,8% Vs 19,4%; p<0,001); vacío quirúrgico en horas
desde el ingreso hasta la primera intervención (3,23±1,36 Vs 4,84±2,09;
p=0,004); vacío quirúrgico en días desde la cirugía índice hasta la reintervención
(5,15±1,62 Vs 9,74±7,26; p=0,013) y el número de reintervenciones (61,5% Vs
100%; p<0,001).
Conclusiones: La edad, el estado civil, los vacíos quirúrgicos, el valor de
hemoglobina preoperatoria, el valor de leucocitos, el número de
reintervenciones, IMC, el ingreso a UCI y el diagnóstico de intervención
quirúrgica son los factores asociados a mortalidad en pacientes con
reintervenciones quirúrgicas abdominales de emergencia.
