Abstract: El estudio se planteó demostrar si las enfermedades crónicas están asociadas a apendicitis
aguda complicada en adultos y se generó la pregunta de investigación: ¿Existe asociación entre
las enfermedades crónicas (diabetes, hipertensión y asma) y la apendicitis aguda complicada en
adultos?.
Se sistematizó el conocimiento relacionado a enfermedades crónicas (diabetes mellitus,
hipertensión arterial y asma) y apendicitis aguda complicada en adultos. Se realizó una
búsqueda sistematizada en Pubmed, Web of Science y Scopus encontrándose 412 estudios, los
cuales fueron evaluados primero por título y resumen, y finalmente a texto completo para las 3
patologías. Luego del proceso de selección, solo quedaron 02 estudios para diabetes por
cumplir con los criterios de inclusión. Se identificó además el riesgo de sesgo utilizando el
instrumento de ROBINS-1.
Al evaluar la síntesis cuantitativa de los resultados en el caso de diabetes no se encontró
significancia estadística en el estimado global (OR:2.11, IC95: 0.82-5.47), observando
heterogeneidad alta mayor al 60%. La calidad de la certeza de la evidencia es baja por el nivel
de inconsistencias observadas por la alta heterogeneidad, imprecisión de intervalos de
confianza y riesgos de sesgo.
No se encontró asociación entre la diabetes y la apendicits aguda complicada. Futuros estudios
deben ser propuestos considerando tamaños muestrales mayores, ajuste por confusores y
distribución homogénea entre grupos.
The study set out to demonstrate whether chronic diseases are associated with complicated acute appendicitis in adults and the research question was generated: Is there an association between chronic diseases (diabetes, hypertension and asthma) and complicated acute appendicitis in adults? Knowledge related to chronic diseases (diabetes mellitus, hypertension and asthma) and complicated acute appendicitis in adults was systematized. A systematic search was carried out in Pubmed, Web of Science and Scopus, finding 412 studies, which were first evaluated by title and abstract, and finally in full text for the 3 pathologies. After the selection process, only 02 studies for diabetes remained to meet the inclusion criteria. The risk of bias was also identified using the ROBINS-1 instrument. When evaluating the quantitative synthesis of the results in the case of diabetes, no statistical significance was found in the global estimate (OR: 2.11, CI95: 0.82-5.47), observing high heterogeneity greater than 60%. The quality of certainty of the evidence is low due to the level of inconsistencies observed due to high heterogeneity, imprecision of confidence intervals and risks of bias. No association was found between diabetes and acute complicated appendicitis. Future studies should be proposed considering larger sample sizes, adjustment for confounders and homogeneous distribution between groups.
