Determine if the use of drains is a risk factor for postoperative morbidity in patients undergoing appendectomy for complicated acute appendicitis. Methods: An observational, analytical, retrospective, case-control study was carried out in patients who presented with complicated acute appendicitis, and who had undergone an appendectomy at the Virgen de la Puerta Essalud High Complexity Hospital. The sample evaluated was 180 patients and clinical, laboratory and surgical data were collected from the medical records. The bivariate analysis was determined with the risk measure of the odds ratio with its respective 95% confidence interval and the Pearson Chi Square test for its association. The data were analyzed with the Epidat 4.2 software. Results: Of the total number of patients with postoperative morbidity, 72.2% used a drain (36.1% was Laminar, 22.3% Mixed and 13.9% Tubular) and 27.8% did not use a drain. Of the total number of patients without postoperative morbidity, 19.4% used a drain and 80.6% did not use it. The difference in both percentages of drain use is statistically significant (p < 0.05) OR interpretation: “the ratio between the use of drain versus the absence of drain use is 10.77 times greater in patients with postoperative morbidity compared to to patients without postoperative morbidity. This association is statistically significant.” We see that the 95% confidence interval of the OR, the lower limit is greater than 1 and the upper limit is greater than 1, the use of drain is a risk factor for postoperative morbidity. Furthermore, the Pearson Chi Square test confirms that there are sufficient reasons at the 5% level (p < 0.05) to affirm that the use of a drain is associated with postoperative morbidity. 9 Conclusion: The use of drains is a risk factor for postoperative morbidity in patients undergoing appendectomy for complicated acute appendicitis and prolonged hospital stay, while the intervening variables exposed to the use of drains were not associated with the development of postoperative morbidity
