To demonstrate from the available evidence what the overall survival is in pancreatectomy with vascular resection compared to pancreatectomy without vascular resection in patients postoperatively with pancreatic adenocarcinoma. This comparison will allow us to determine whether vascular resection provides a significant benefit in terms of overall survival. MATERIAL AND METHODS: A systematic review and meta-analysis of the available literature was performed. Primary original articles published between 2006 and 2024 in indexed databases were included. The chosen studies were analytical observational studies that address overall survival in patients with pancreatic adenocarcinoma post pancreatectomy with and without vascular resection. The data were analyzed using statistical software such as Rayyan and RevMan. RESULTS: Thirty-five cohort studies and one case-control study were identified, including 9273 patients, of whom 25.4% underwent pancreatectomy with vascular resection (VR+) and 74.6% underwent pancreatectomy without vascular resection (VR-). Postoperative mortality measured at 30 days revealed that the VR+ group had an 82% higher mortality rate (RR 1.82, 95% CI 1.41-2.35, p = 0.85) compared with the VR- group. On the other hand, VR+ patients showed an improvement in overall survival (OS) at 1 year of 7% (RR 0.93, 95% CI: 0.86 - 0.99, p = 0.03), OS at 2 years of 20% (RR 0.80, 95% CI: 0.69 - 0.92, p = 0.003), OS at 3 years of 28% (RR 0.72, 95% CI: 0.63 - 0.82, p = 0.07) and OS at 5 years of 40% (RR 0.60, 95% CI: 0.47 - 0.76, p = 0.002). CONCLUSIONS: Patients with post-pancreatectomy pancreatic adenocarcinoma who undergo vascular resection (VR+) have an 82% probability of 30 day-postoperative mortality compared to those who do not undergo vascular resection (VR-). Regarding overall survival, VR+ was shown to be associated with an improvement in estimated overall survival at 1 year, which continued to increase at 2 and 3 years, reaching a clinically significant improvement at 5 years in the VR+ group, demonstrating the superior efficacy of this intervention
