OBJECTIVES: To evaluate the effectiveness of analgesia in continuous infusion of Bupivacaine plus Fentanyl via epidural compared to epidural analgesia in boluses with Bupivacaine plus Fentanyl for the treatment of postoperative pain after abdominal hysterectomy. MATERIAL AND METHODS: A cohort study was conducted, 50 patients undergoing elective abdominal hysterectomy entered the study at the High Complexity Hospital Virgen de la Puerta de EsSalud. Two groups were formed, Group 1 = 24 patients who received Bupivacaine plus Fentanyl in continuous infusion via epidural and Group 2 = 26 patients who received bolus of Bupivacaine plus Fentanyl every 4 hours via epidural. RESULTS: The age mean in groups 1 and 2 were 43.78 ± 4.66 and 43.46 ± 5.24 respectively (p > 0.05). The scores of the Visual Aanáloga Scale reported in groups 1 and 2, showed that the scores obtained in group 1 were significantly lower at times 2, 6, 10, 14, 18, 22, 26 and 30 hours respectively (p < 0.001). The incidence of adverse effects and rescue analgesia in groups 1 and 2 were 16.67% vs 61.54% (p < 0.001), RR 0.30: IC 95 [0.12-0.75] and 0% 19.23% respectively (p <0.05). CONCLUSIONS: The peridural route of bupivacaine plus fentanyl in infusion continued to be more effective than the bolus infusion for the treatment of postoperative pain in women going to abdominal histyerectomy.
