To evaluate if infestation with Demodex sp. is associated with severe acne vulgaris in outpatient settings in Hospital Regional Lambayeque. Materials and methods: We perform acase control study in 46 patients with severe acne vulgaris and 92 patients with non-severe acne vulgaris. Severe acne vulgaris was defined as a grade ≥ 3 in the EGAE scale, the infestation with Demodex sp. was determined by a cutaneous superficial biopsy and infestation diagnostic was stablished as more than 5 mites per cm2. Results: 60,9% of study subjects were male, with a median age of 18 years and a interquartile range between 15 and 20 years, and they lived in a urban setting (81,9%) and the frequency of infestation with Demodex sp. was 29,7%. In the bivariate analysis, severe acne vulgar was associated with the infestation with Demodex sp. (p=0,001), sex (p=0,003), urban settings (p=0,015), familiar antecedent of acne (p=0,045) and type of skin (p<0,001). In the multivariate analysis the infestation with Demodex sp. could produce 4,17 times the risk of having severe acne vulgar (IC: 1,61 – 10,82, p=0,003) adjusted by sex, urban setting, parental antecedents of acne, and oily skin. Conclusion: Infestation with Demodex sp. was associated to severe acne vulgar.
