To compare the efficacy of laparoscopic excision versus laparoscopic ablation for the treatment of pain and infertility in patients with endometriosis. Materials and methods: A systematic review of randomized clinical trials was performed using the Cochrane CENTRAL, ScienceDirect, Pubmed, ClinicalKey, and ClinicalTrials.gov databases. Study selection, risk of bias and data extraction were performed by two authors independently. The results for pain efficacy was a decrease in visual analog scale scores for dysmenorrhea, dyspareunia, dyschecia, and pelvic pain. The results for efficacy in infertility were the number of pregnancies reported during follow-up. Mean differences in each study were extracted and used for meta-analysis. Results: 3 studies were selected for meta-analysis. The excision technique showed a significant difference in pain reduction (MD = 1.87; 95% CI; 0.64-3.11) at 6 months. No significant differences were found in the reduction of dyspareunia, dyschecia or pelvic pain. In the narrative description, no significant differences were found in the number of reported pregnancies. Conclusions: The excision technique is more effective in reducing dyspareunia at 6 months compared to the ablation technique. No differences were found in terms of the decrease in pelvic pain, dyspareunia, dyschecia or in the number of reported pregnancies.
