Objective: Determine if the C-reactive protein/albumin ratio has value as a predictor of appendiceal perforation. Material and methods: A retrospective sectional study of diagnostic tests was carried out in which 185 patients with acute appendicitis were included, according to selection criteria which were divided into 2 groups: perforated or non-perforated; calculating sensitivity, specificity, positive predictive value and negative predictive value. Results: The sensitivity and specificity of the C-reactive protein index. /Albumin as a predictor of appendiceal perforation was 82% and 74% respectively (p<0.01), the positive and negative predictive value of the C-reactive protein/albumin Index as a predictor of appendiceal perforation was 64% and 88% respectively, the area under the curve of the Index c-reactive protein/albumin for the prognosis of perforation in acute appendicitis corresponds to a prognostic accuracy of 82%. No significant differences were observed regarding the variables age, sex, anemia, hyponatremia, hyperazoemia, leukocytosis, diabetes or hyperlactatemia between patients with perforated or non-perforated appendicitis (p>0.05). Conclusion: The C-reactive protein/albumin ratio has value as a predictor of appendiceal perforation
