Objective: To determine the risk factors for early mortality in patients with chronic kidney disease (CKD) on hemodialysis. Methodology: A retrospective cohort was analyzed in 234 patients with CKD who started hemodialysis in the period 2019 to 2022. The following risk factors were studied: age ≥ 60 years, glomerular filtration rate > 7 mL/min/1.73 m2, late referral to nephrology, hypoalbuminemia, anemia, Charlson index ≥ 3 points, initiation of hemodialysis via central venous catheter and diabetic etiology; The outcome evaluated was mortality within the first 90 days after starting hemodialysis. Bivariate and multivariate analysis was performed with the Cox regression model calculating the hazard ratio (HR) with 95% confidence intervals (CI) and p value < 0.05. Results: Of the 234 patients who started hemodialysis, early mortality occurred in 32 (13.635%) patients. Risk factors for early mortality were age ≥ 60 years (HR: 4.853; 95% CI: 1.475-15.968; p= 0.009), hypoalbuminemia (HR: 2.386; 95% CI: 1.031-5.520; p= 0.042) and Charlson index ≥ 3 points (HR: 7.343; 95% CI: 1.020-54.365; p= 0.045). Conclusion: The risk factors for early mortality in patients with chronic kidney disease on hemodialysis were age ≥60 years, hypoalbuminemia, and Charlson Index ≥3 points
