Abstract: OBJETIVO: El propósito de este estudio fue investigar la relación entre la diabetes
mellitus tipo 2 y la apendicitis aguda complicada en pacientes atendidos en el
Hospital Regional Docente de Trujillo.
MATERIAL Y MÉTODOS: En este estudio observacional de casos y controles se
realizó un análisis retrospectivo de los datos de 345 pacientes con apendicitis
aguda almacenados en el archivo del HRDT. La variable independiente fue el
diagnóstico de DM2 previa a la hospitalización y la dependiente fue apendicitis
aguda complicada. Se analizó el odds ratio (OR) y su correspondiente intervalo de
confianza del 95% (IC).
RESULTADOS: La frecuencia de pacientes con diagnóstico previo de diabetes
mellitus tipo 2 fue mayor en el grupo de pacientes con apendicitis aguda
complicada, con 9,3%; comparado a los que presentaron apendicitis aguda no
complicada y diabetes mellitus tipo 2, cuya frecuencia fue 1,3%. La prueba de chi
cuadrado reveló diferencia significativa: p<0.001, IC 95% (2,18 – 29,21), y un OR
de 7,98. En el análisis multivariado se encontró además significancia estadística
entre apendicitis aguda complicada y el sexo masculino (p=0.009 OR), puntaje de
Alvarado >4 (p=043), glucemia >126 (p=003), la presencia de apendicolito
intraluminal (p=001) y duración de síntomas >48 horas (p=001).
CONCLUSIÓN: La diabetes mellitus tipo 2 es factor de riesgo para apendicitis
aguda complicada en pacientes atendidos en el Hospital Regional Docente de
Trujillo

The chi square test revealed a significant difference: p<0.001, 95% CI (2.18 – 29.21) and an OR of 7,98. Statistical significance was also found with the male sex (p=0.009), Alvarado score >4 (p=043). , blood glucose >126 (p=003), the presence of intraluminal appendicolith (p=001) and duration of symptoms >48 hours (p=001).
