To determine the factors associated with postoperative fistula in adult patients undergoing gastrointestinal oncological surgery. Material and methods: A retrospective cohort study was conducted from January to December 2023, including all adult patients who underwent surgery in the oncological surgery department of the Hospital de Alta Complejidad Virgen de la Puerta in Trujillo, Peru. A total of 123 patients were included. The variables investigated were: sex, age, hypertension, type 2 diabetes mellitus, preoperative malnutrition, frailty, surgical technique (laparotomy, laparoscopy), and preoperative serum albumin less than 3 mg/dl. Results: preoperative malnutrition with an RR: 4.658, CI: 1.49 - 14.51 with p: 0.0009 and serum albumin less than 3 mg/dl preoperatively with an RR: 4.807, CI: 2.61 - 8.82 with p: 0.00003 showed a statistically significant association in the multivariate analysis. Similarly, through bivariate analysis, it was shown that preoperative malnutrition (p=0.0009) and serum albumin <3.0 mg/dl (p=0.00003) were factors associated with the development of postoperative fistula in adult patients undergoing gastrointestinal cancer surgery. Conclusions: In this study, the factors associated with postoperative fistula development in patients undergoing gastrointestinal cancer surgery were preoperative malnutrition and hypoalbuminemia (serum albumin <3 mg/dL).
