To evaluate the efficacy of the lymphocyte platelet index (LPI) as inflammation marker in the diagnosis of premature rupture of membranes (PROM) in patients with preterm pregnancy. Method: Diagnostic test study that included 375 patients with preterm pregnancy treated between 2018 and 2019 at the Víctor Lazarte Echegaray Hospital, stratified according to the diagnosis of PROM. The platelet-lymphocyte index was calculated for each patient, the means for each comparison group, and a ROC curve was plotted to assess the overall diagnostic performance of this marker. The optimal cut off point and the classic parameters of diagnostic yield were calculated for this IPL value. Results: The mean IPL in the group with PROM was significantly higher than the group without PROM (125.6 vs 119.3) (p<0.001). The ROC curve showed an area under the curve of 0.673 (p=0.001). The optimal cut-off point for the IPL was 119, which showed a sensitivity of 71.5%, a specificity of 58.5%, and positive and negative predictive values of 52.8 and 73.6%, respectively. Conclusion: The platelet-lymphocyte as index inflammation marker is ineffective in the diagnosis of premature rupture of membranes in patients with preterm pregnancy
