To demonstrate that the high leukoglycemic index is a predictor of in-hospital mortality in patients with acute ischemic stroke of the Neurology Department at Belén Hospital of Trujillo in the period from January 2014 to December 2018. MATERIAL AND METHOD: Observational, analytical, cohort and retrospective research, conducted at Belén Hospital of Trujillo, with a census sample of patients hospitalized in the Neurology department with diagnosis of acute ischemic stroke in the period from January 2014 to December 2018, who met the selection criteria set out, and from whom the value of leukocytes and serum glycemia was obtained within 24 hours after admission, documented in the medical records. RESULTS: 281 clinical histories were analyzed, where 18 were of deceased patients, of which 56% died with leukoglycemic index greater than or equal to 1600, this result being statistically significant in the bivariate analysis (RR: 6.56; CI: 95% (2.74-15.70); p <0.001) and multivariate (RRa: 6.33, CI: 95% (1.35 - 29.64); p = 0.019). In addition, in the multivariate analysis, only age greater than or equal to 80 years (RRa: 5.33, IC: 95% [1.12 – 25.38], p=0.036), admission awareness disorder (RRa: 8.36, IC: 95% [1.83 – 38.06], p=0.005) and in-hospital pneumonia (RRa: 41.98, IC: 95% [5.69 – 309.92], p<0.001) were relevant. CONCLUSION: The high leukoglycemic index is a predictor of in-hospital mortality in patients with acute ischemic stroke of the Neurology Department at Belén Hospital of Trujillo in the period from January 2014 to December 2018
