Introduction: Acute perforation of the appendix is one of the complications of appendicitis that is associated with increased morbidity, and therefore is considered a surgical emergency. Patients who underwent an appendectomy for perforated appendicitis are more likely to develop wound infection or abscess, the use of drains could prevent these complications; However, recent studies do not justify this therapeutic measure. Objective: To determine if the use of a Penrose abdominal drain is a risk factor for superficial surgical site infection in acute perforated appendicitis at the Belén Hospital in Trujillo. Material and methods: An analytical, retrospective cohorte study was carried out in which 84 patients with a diagnosis of perforated appendicitis were included, according to selection criteria which were divided according to the presence or absence of Penrose abdominal drain use, the chi square and the relative risk statistic were calculated. Results: The frequency of surgical site infection in patients with acute perforated appendicitis using the Penrose abdominal drain was 26%; The frequency of superficial surgical site infection in patients with acute perforated appendicitis who were not users of the Penrose abdominal drain was 7%; the use of abdominal drain as a risk factor for superficial surgical site infection in acute perforated appendicitis, with a relative risk of 3.6, which was significant (p<0.05). No differences were observed regarding the frequency of anemia, diabetes, hypoalbuminemia, hyponatremia, plateletopenia, elevated CRP and shock between patients with or without SSI (p>0.05). There were also no differences in the variables of age, sex and comorbidities such as HBP and type 2 DM between patients with perforated appendicitis who used a Penrose drain compared to those who did not use it (p>0.05). Conclusion: The use of Penrose abdominal drain is a risk factor for superficial surgical site infection in acute perforated appendicitis at the Belén Hospital in Trujillo
