Intraligamentous implantation is a very rare location within the already few intraabdominal ectopic pregnancies. Without an early diagnosis, the therapeutic options are reduced, as is the life expectancy of the pregnant woman. We present the case of a 26-year-old pregnant woman without prenatal controls, with a history of right ovarian cystectomy 5 years ago, who attended emergencies due to gynecorrhagia and abdominal pain of one week of evolution. Laboratory reports hemoglobin of 9.6 g / dL and leukocytosis; the rapid test for COVID-19 was negative. Abdominal ultrasound showed a viable 12-week single gestation. She was admitted to the operating room with a probable diagnosis of acute appendicitis; finding hemoperitoneum and tumor with presence of fetus. After the intervention, she had a favorable evolution and was discharged.
