Objective: Prove that the C-reactive protein / albumin ratio is a better predictor of mortality that the neutrophil / lymphocyte ratio in cirrhotic patients with spontaneous bacterial peritonitis at the Trujillo Regional Teaching Hospital in the period March 2013- July 2018. Material and methods: A retrospective sectional study of diagnostic tests was carried out in which 59 cirrhotic patients with spontaneous bacterial peritonitis were included, according to selection criteria which were divided into 2 groups: deceased or survivors; calculating the area under the curve for the C-reactive protein / albumin ratio and the neutrophil / lymphocyte ratio. Results: The average age and time of illness and the frequency of the functional class Child Pugh C and of hyponatremia were significantly higher in the group of cirrhotic patients with spontaneous bacterial peritonitis who died than in the survivors. The sensitivity, specificity, positive and negative predictive value of the neutrophil / lymphocyte ratio as a predictor of mortality in cirrhotic patients with spontaneous bacterial peritonitis was 81%; 88%; 72% and 93% respectively. The sensitivity, specificity, positive and negative predictive value of the PCR / serum albumin ratio as a predictor of mortality in cirrhotic patients with spontaneous bacterial peritonitis was 88%; 84%; 67% and 95% respectively. The prognostic accuracy of the neutrophil / lymphocyte ratio in cirrhotic patients with spontaneous bacterial peritonitis was 89%. The prognostic accuracy of the CRP / serum albumin ratio in cirrhotic patients with spontaneous bacterial peritonitis was 90.6%. Conclusion: The C-reactive protein / albumin ratio and the neutrophil / lymphocyte ratio alike are predictors of mortality in cirrhotic patients with spontaneous bacterial peritonitis at the Trujillo Regional Teaching Hospital in the period March 2013- July 2018.
