Introduction: respiratory distress syndrome is a very common clinical sign in prematurity, its severity may be related to the acidic disorder. Objective: to establish the association between the syndrome of severe respiratory distress and the acidic disorder in preterm newborns. Method: Historical cohort study where 160 medical records (80 with and without severe respiratory distress syndrome) of premature newborns in the neonatology area of the Belen de Trujillo Hospital between 2010 and 2018 were reviewed. Relative Risk was calculated (RR) for the bivariate analysis and the multivariate analysis was performed by logistic regression to establish the independent risk factors. Results: There was no significant difference in gestational age (30.9 vs 31.4 weeks, p> 0.05), nor in the sex of the neonate. The acidemic disorder occurred in 51% of premature infants with severe respiratory distress syndrome, and was an independent risk factor when adjusting the intervening variables (RRa: 2.13) Conclusion: Acidemic disorder is associated with severe respiratory distress syndrome.
