Objective: To analyze the effect of probiotics on the clinical improvement of children with autism spectrum disorder (ASD) through a systematic review and meta-analysis of previous clinical trials. Methods: A comprehensive systematic review of the current scientific literature was conducted using databases such as PubMed, Scopus, Cochrane Library, Embase, and Web of Science. Clinical studies evaluating the impact of probiotics on symptoms and quality of life in children with ASD were identified and collected, using the Social Responsiveness Scale (SRS), the Autism Treatment Evaluation Checklist (ATEC), assessment of diarrhea through the Gastrointestinal Severity Index (GSI), and the incidence of adverse effects. Results: Nine randomized clinical trials investigating the efficacy of various probiotics in children with ASD were included. The studies, conducted in different countries, had sample sizes ranging from 10 to 71 participants, with a predominance of males in most. The probiotics used were diverse and specific, while placebos varied in composition. Follow-up periods ranged from 4 weeks to 8 months. A beneficial effect was found in improving social responsiveness and reducing diarrhea severity, with statistical significance; however, there was variability in the magnitude of effects and heterogeneity among studies. No statistically significant difference was observed in adverse effects between probiotics and placebo. Conclusion: Overall findings suggest benefits in social responsiveness, improvement of gastrointestinal symptoms, and reduction of diarrhea severity
