OBJECTIVE: To determine whether in postoperative patients with acute appendicitis, appendiceal base involvement is a risk factor for intra-abdominal complications. MATERIALS AND METHODS: An observational, analytical, retrospective cohort study was performed, 360 patients with acute appendicitis attended at the Trauma and General Surgery Service of the Regional Teaching Hospital of Trujillo during the period from January 2018 to December 2022 were evaluated; cohorts of 180 patients each were formed according to the presence or absence of appendicular base involvement to evaluate the outcome postoperative complications. RESULTS: In this study, it was found that 180 patients had a compromised appendicular base, while 180 patients did not. It was observed that 18.33% of patients with a compromised appendicular base and 10.56% of patients without it experienced postoperative complications, with a p-value of 0.034. Surgical wound infection was the most common complication in both groups, with a higher number of cases in the group with a compromised appendicular base (12.22%) compared to the group without it (7.78%). Furthermore, a significant difference was observed in the incidence of intraabdominal abscesses, which were higher in the group with a compromised appendicular base (4.44%) compared to the group without it (2.78%). CONCLUSIONS: Compromised appendiceal base in patients with acute appendicitis are at increased risk for intraoperative and postoperative complications.
