Objective: To check the high histological grade on the prognosis of recurrence in patients with soft tissue sarcomas. Patients and Methods: A cohort study was conducted, evaluated 68 patients diagnosed with soft tissue sarcomas from the Regional Institute of Neoplastic Diseases ““Dr. Luis Pinillos Ganoza ““IREN NORTE Trujillo-Peru, who met the inclusion criteria. Results: Of a total of 68 patients with soft tissue sarcoma, 34 patients with high grade histological and 34 low grade. The average age was 54,2 ± 21,2 years in patients with recurrence (group 1) and 54,1 ± 21,3 years without recurrence (group 2) (p =0,865); male sex was present in 65% and 35% of cases in group 1 and 2 respectively (RR de 1,65 IC 95% [0,99 - 2,76]; p=0,036). In the histopathological study, the presence of necrosis was present in 68% of cases with recurrence (RR de 1,56 IC 95% [1,09 - 2,43]; p= 0,041), the high histological grade was present in 71% of the cases of group 1 (RR of 1, 84 IC 95% [1, 14 - 2, 98], p = 0,007), In addition, 34% (23 patients) of the total cases were Liposarcoma and the lower limbs were affected in 62% (42 patients). Conclusions: The high histological grade had twice the risk of recurrence of soft tissue sarcoma compared to those with low histological grade; Liposarcoma in the majority of cases, and affection were presented to the lower limbs.
