Objective: Determine if maternal age, parity, fetal macrosomia are risk factors for severe perineal sprain. Material and methods: A case and control study was conducted. We reviewed 87 clinical records of patients whose vaginal births were attended at the Belén Hospital in Trujillo, from January 2013 to December 2016, of which 29 had severe perineal sprain and 58 had not. Results: We found that the nulliparity (X2 = 6.415, a value p = 0.011, an OR = 3.9, a 95% CI: 1.306 - 11.642 and a contingency coefficient of 0.262) and fetal macrosomia (X2 = 4.633; p = 0.031, an OR = 3.279, a 95% CI: 1.075 - 9.997, and a contingency coefficient of 0.225) are risk factors for severe perineal sprain. No statistical significance was found between maternal age >35 years and <19 years, multiparity and severe perineal sprain. Conclusions: It was determined that nulliparity and fetal macrosomia are risk factors for severe perineal sprain.
