OBJECTIVE: The objective was to determine if the neutrophil-to-lymphocyte ratio is a marker of inflammation useful in predicting the diagnosis of complicated acute appendicitis. MATERIAL AND METHOD: An observational, analytical study of diagnostic tests was conducted during the period from November 1, 2017 to February 28, 2018 in two hospitals in the city of Trujillo; we evaluated 375 patients diagnosed with acute appendicitis who were operated during this period, finding a prevalence of complicated acute appendicitis (CAA) of 54.13%. RESULTS: The age in the groups with and without AAC were 32.75 ± 11.21 and 31.46 ± 10.83 years respectively (p> 0.05); the proportion of males in each group was 59.61% and 65.12% respectively (p> 0.05). The leukocyte, absolute neutrophil and l ymphocyte count values and the NLR in the groups with and without AAC were the leukocytes 16156.15 ± 3467.44 and 14427.08 ± 2314.17 (p <0.001); neutrophils 12911.95 ± 3383.98 and 11237.52 ± 2144.80 (p <0.001); lymphocytes 1457.92 ± 819.73 and 1849.18 ± 668.85 (p <0.001) and NLR 12.15 ± 8.66 and 7.38 ± 5.00 (p <0.001) respectively. Taking as a cut- off point the INL value of 5.74 for AAC, a sensitivity of 81.77% was found, specificity of 45.93%, positive predictive value of 64.09%, negative predictive value 68.10% and a diagnostic accuracy of 72%. CONCLUSIONS: We suggest that preoperative NLR is a useful inflammatory marker to differentiate between complicated and uncomplicated acute appendicitis, and can be used as a complement in the clinical examination.
