Objective: To demonstrate if decreased HDL cholesterol is a risk factor for organ failure in patients with acute pancreatitis in the Bethlehem Hospital of Trujillo Material and Methods: An analytical, observational, retrospective, case and control study was carried out. The study population consisted of 136 patients with severe acute pancreatitis; which were divided into 2 groups: with organ failure or without it; the odds ratio and the chi square test were calculated. Results: The frequency of decreased HDL cholesterol in patients with severe acute pancreatitis and organ failure was 35%. The frequency of decreased HDL cholesterol in patients with severe acute pancreatitis and without organ failure was 19%. Decreased HDL cholesterol is a risk factor for organ failure in severe acute pancreatitis with an odds ratio of 2.3 which was significant (p <0.05). In the multivariate analysis, the significance of the risk for the decreased HDL variables, hemoconcentration and elevated CRP was verified with respect to the outcome of organ failure in severe acute pancreatitis. Conclusions: Decreased HDL cholesterol is a risk factor for organ failure in patients with acute pancreatitis at the Belén de Trujillo Hospital
