Introduction: Cranioencephalic trauma is one of the most dangerous conditions for the life of the person, decompressive craniectomy is a therapeutic intervention but involves risks, the factors that are associated with mortality are variable and continue in research. Objective: To demonstrate that pre-surgical factors are associated with mortality in patients undergoing decompressive craniectomy due to severe head trauma. Method: A case-control study in which 84 medical records (42 cases and 42 controls) of patients diagnosed with severe head trauma undergoing decompressive craniectomy were reviewed in the neurosurgery service of the Víctor Lazarte Echegaray Hospital between 2011-2017. The Odds Ratio (OR) was calculated for the bivariate analysis and the multivariate analysis was performed using logistic regression to establish the associated factors. Results: Glasgow <8 points (ORa: 7.42), hypotension (ORa: 5.13), bilateral mydriasis (ORa: 5.35), and type 2 diabetes mellitus (ORa: 4.97) presented association in the bivariate analysis and were maintained after adjust to other variables as factors associated with mortality in patients undergoing decompressive craniectomy due to severe head trauma. Conclusion: Glasgow <8 points, hypotension, bilateral mydriasis and type 2 diabetes mellitus are factors associated with mortality in patients undergoing decompressive craniectomy due to severe head trauma.
