OBJECTIVE: To assess whether preoperative neutrophil-to lymphocyte ratio is a predictor of severe acute cholecystitis in patients of Hospital Regional Docente de Trujillo during the period January 2012 to July 2015. MATERIAL AND METHODS: We conducted an observational, analytical study of diagnostic tests that evaluated 320 patients operated for acute cholecystitis, which were divided into two groups, Group I (48 patients with severe acute cholecystitis) and Group II (272 patients without severe acute cholecystitis). RESULTS: The mean age in groups I and II were 61,52 ± 14,48 years and 50,44 ± 16,09 years, respectively (p < 0,001); 37,5% were male in the group I and 38,97% in group II (p > 0,05). The mean white blood cell count, neutrophils, lymphocytes and neutrophil-to lymphocyte ratio in patients with and without this condition were: WBC: 17 968.33 ± 3 931.85 and 15 294.04 ± 4031.45 (p < 0,001); neutrophil: 81,44 ± 6,52 and 70,13 ± 5,34 (p < 0,001); lymphocytes: 14,42 ± 5,35 and 22,63 ± 3,76 (p < 0,001) and the neutrophil-to lymphocyte ratio: 6,47 ± 2,55 and 3,23 ± 0,84 (p < 0,001). Regarding the performance of the NLR, with to cut a 3-point, the sensitivity, specificity, PPV and NPV were 83,33%, 71,69%, 34,19% and 96,06% respectively; an area under the curve of 0,91. CONCLUSIONS: The neutrophil-to lymphocyte ratio is useful in predicting severe acute cholecystitis with a diagnostic accuracy of 91%.
