Determine the ability of neutrophil lymphocyte index to predict bacteremia in critically ill patients in Trujillo Belen Hospital. Material and methods: Retrospective, observational, diagnosis test study to aplicate in 115 critically ill. We calculated the sensibility, specificity, positive and negative predictive value to mortality in acute coronary syndrome. We calculated the chi square test and the area under the curve. Results: The best cut off value to neutrophil lymphocyte index to predict bacteremia was 5.5 and sensibility, specificity, positive and negative predictive value was 71%, 82%, 75%, 79% respectively . The under the curve area were 0.80. Conclusions: The best cut off value of neutrophil lymphocyte index were 5.5 and this offers the better predictive value with a great estadistical significance to bacteremia in critically ill patients. The under the curve area offer an intermedium degree of exactitude
