Objective: Determine if the elevated neutrophil-lymphocyte proportion is a factor associated with severe COVID-19. Material and Method: An observational, analytical, and cross-sectional study was conducted. The sample was made up of 183 patients with a confirmed case of COVID-19 who were treated in the emergency area of the Albrecht Hospital – EsSalud in Trujillo from May 2020 to December 2021 and who met the selection criteria. This information was recorded on a data collection sheet. Pearson's Chi-square test, Student's T test, and multivariate Poisson regression analysis were used for statistical analysis. Results: The frequency of severe COVID-19 was higher in patients with an elevated neutrophil-lymphocyte proportion compared to patients with a non elevated neutrophil-lymphocyte proportion (76.3% vs 27.3%), demonstrating that the elevated neutrophil-lymphocyte proportion is a factor associated with severe covid-19 (p<0.05). The intervening variables associated with severe COVID-19 were age (p < 0.001), sex (p = 0.008), diabetes mellitus (p = 0.022), CRP ≥ 10mg/l (p = 0.013), D-dimer > 1 ug/ml (p=0.002) and Chest X ray/CT compatible with pneumonia (p < 0.001). The intervening variables not associated with severe COVID-19 were high blood pressure, obesity, cardiovascular disease and temperature > 38°C. The multivariate analysis showed that the elevated neutrophil-lymphocyte proportion and chest X ray/CT compatible with pneumonia were significant (p<0.05). Conclusion: The neutrophil-lymphocyte proportion is a factor associated with COVID-19
