Objective: To evaluate sexual function after vaginal and abdominal hysterectomy. Materials and Methods: An observational, analytical prospective cohort study was conducted in the Gynecology Service of the Bagua Support Hospital ““Gustavo Lanatta Luján““ during the year 2022, with a total of 90 patients between 18 and 50 years old, sexually active and with current sexual partner, who did not present sexual dysfunction before surgery, which were distributed in two cohorts of 45 patients each (abdominal and vaginal hysterectomy cohort). Results: The mean age in the vaginal and abdominal hysterectomy cohort were 43.38 ± 7.08 and 45.29 ± 4.43 (p = 0.129) respectively; the use of psychotropic drugs, the presence of menopause, the presence of atrophy and the etiology of hysterectomy, had no statistically significant differences. The proportion of female sexual dysfunction in the vaginal and abdominal hysterectomy cohort were 31.11% vs 44.44% (p = 0.192), with a relative risk of 0.74 and 95% CI [0.47 - 1.18]. Conclusions: Vaginal post hysterectomy sexual function is not different from abdominal post hysterectomy sexual function
