Background: In the prevention of preeclampsia, different measures have been tried, such as supplementation with different vitamins, including vitamin D, C and E and multivitamin complexes, however, until now, its use is controversial. Aim: To determine if vitamin supplementation is effective to prevent preeclampsia. Methods: A systematic review was conducted identifying and extracting data from five randomized clinical trials and two observational studies comparing vitamin supplementation versus placebo or only medication from routine antenatal care. Results: In the prevention of preeclampsia: analysis of clinical trials: RR of 0.59 with CI of 0.37 - 0.94, I2 of 44% and p = 0.02. In the analysis of observational studies: RR of 0.31 with CI of 0.17 - 0.56, I2 of 61% and p = 0.0001. In the incidence of preterm birth: analysis of clinical trials: RR of 0.75 with CI of 0.45 - 1.25, I2 of 39% and p = 0.27. In the analysis of observational studies: RR of 0.37 with CI of 0.18 - 0.75, I2 of 0% and p = 0.006. In the incidence of low birth weight: analysis of clinical trials: RR of 0.72 with CI of 0.46 – 1.12, I2 of 39% and p = 0.14. In the analysis of observational studies: RR of 0.57 with CI of 0.25 – 1.30, heterogeneity not applicable and p = 0.18. Conclusions: Vitamin supplementation with Vitamin D, or Vitamin C and E prevents preeclampsia.
