OBJECTIVE: The main objective of this thesis project is to determine which are the predictive factors of success of Vaginal Birth in women with Previous Caesarean Section. MATERIAL AND METHOD: An observational - analytical retrospective cohort study was carried out with 2 study groups: exposure cohort (cesarean section in last birth) and control cohort (vaginal birth in last birth). The affiliation data will be used, as well as the prenatal records found in the medical records for the diagnoses of “Vaginal birth” with a history of previous cesarean section. The variables to be studied will be: Maternal age, multiparity, cephalic presentation, fetal weight by ultrasound, previous vaginal delivery, intergenic period in years, prenatal controls, BMI at the end of pregnancy. RESULTS: 1803 pregnant women were selected divided into 2 study cohorts, 766 belonging to the exposure cohort and 1037 for the control cohort. When performing the multivariate analysis, it was found that the probability of successful delivery is 6 times higher in pregnant women who had a vaginal delivery compared to pregnant women with a history of cesarean section in the last delivery (OR: 6.556; CI: <5.321, 8.078>; P=0.000). However, 31.5% of successful vaginal births had a cesarean section in the last birth (n=229). Therefore, it was decided to perform a subgroup analysis, finding that having a pregnant woman in labor <27 years of age and who is not obese improves the probability of successful vaginal delivery in women with a history of cesarean section in the last birth
